VETERAN'S HOME CARE

Veteran’s Home Care is funded by the Department of Veteran’s Affairs (DVA) and provides support for veterans who live in their own home. This support can include assistance with cleaning, shopping, personal services and basic home maintenance.

If you require assistance through this scheme you need to contact the VHC Assessment Agency on 1300 550 450. The Assessment Agency will determine your eligibility and need for care and then contact a care provider to deliver the required services. (This may or may not be our service.)

The amount of care and the type of care/support provided is determined by the VHC Assessment Agency.

COMMUNITY AGED CARE PACKAGES

Community Aged Care Packages are funded through the Department of Health and Ageing and provide support for seniors who live in their own home. This flexible support can include assistance with personal care, medical appointments, accompanied shopping and cleaning.

Eligibility for CACP’s is determined by your local Aged Care Assessment Team (ACAT). Assessment can be arranged through you GP, local hospital or by contacting the ACAT team direct.

If you are assessed as requiring a CACP package, you need to contact our office (5451 0799) to arrange an interview. Our coordinator will then visit you to determine your care needs and the amount of support/care we can provide.

Please note that we can only deliver CACP care/support within the Maroochydore / Noosa / Caloundra areas.
